PCMH is an innovative, team-based approach to providing enhanced primary healthcare created by the American Academy of Pediatrics in 1967. Sandhills Pediatrics has been recognized by the National Committee for Quality Assurance as a PCMH since May 27, 2010.

PCMH means team-based care.

We partner with patients, parents, and the entire health care team. We follow evidence-based guidelines for medical care to coordinate the best health care services possible.  We care for the “whole child” across multiple settings, including mental health. We develop plans for self-management by providing you and your child with strategies and tools to easily manage chronic illnesses, including asthma, diabetes, and ADHD. If your child misses an appointment with a specialist, a lab test, or an imaging test, our team will follow-up with you. You can help us coordinate your child’s care by asking specialists, emergency departments, and urgent cares to fax us summaries of your child’s visit to (910)692-3651.

We expect you to select a primary care provider at Sandhills Pediatrics, but you are always free to see any of our providers.  We are committed to make available to you a personal physician who provides for all of your health care needs and coordinates your care across all settings, including the medical office, hospital, clinics, mental health, testing facilities and other places where you receive health care.  We will enter your primary care provider in our computer system to make appointment scheduling easier.

PCMH means improved access.

We work diligently to increase availability. We opened a new office in Raeford in 2011 and doubled our clinic space in Seven Lakes in 2012.  In 2014, we added early bird walk-in clinic. We continue to recruit excellent pediatricians and pediatric nurse practitioners to meet your child’s healthcare needs. We added a child psychiatrist in 2013 and a psychiatric nurse practitioner in 2014 to enhance our ability to provide excellent mental healthcare.  Since then we have continued to steadily expand our mental health team.   In July 2014, we moved our Raeford office into the former Jordan clinic, doubling our space to better meet demand. 

 We encourage you to use your PCMH for comprehensive, coordinated primary care. We offer appointments weekday evenings until 7pm, Saturday mornings and Sunday afternoons, and most holidays at our Southern Pines office.

PCMH means improved communication.

Our office phone triage nurses can help you with medical questions 8am-7pm Monday through Friday. Our enhanced website offers a home triage application “Is my child sick?”created by experts from the American Academy of Pediatrics.  Let us know if you would like to discuss self-management of chronic illness, including asthma, food allergy, and diabetes.

We are most effective when we have complete information about care your child receives outside Sandhills Pediatrics. At each visit, let us know all medications your child is taking, including over-the-counter medications, supplements and vitamins. Let us know if your child has seen a specialist or other healthcare provider and if there are any changes in his health history or status, including hospitalization, urgent care or emergency department visits. We need to know results of any medical tests ordered by other providers. Let us know if your child has received any vaccines elsewhere, including flu vaccine.  

If you are a new patient transferring in, please call 910-692-2444 and enter #202.  This will transfer you to our incoming medical records department where Juanita can assist you.

Always make sure we have correct phone numbers and addresses for your child. You can choose phone, email or text appointment reminders. If you would like text reminders, please tell a check-in or check-out staff member.  Then text “SHPEDS” to 622-622 to opt-in. You will get a response “welcoming” you to text reminders.

We now have a patient portal, where you can securely email us non-urgent appointment requests, request prescription refills, read test results, and even print a copy of your child’s immunization record!  Select “My Chart” from the header bar at the top of the webpage.  You can sign up for access during your next visit to the office.

PCMH means help after hours.

Our Sandhills Pediatrics family is available to your family 24/7. When our office is closed, specialized pediatric triage nurses based at UNC-Chapel Hill answer urgent calls and page our on-call Pediatrician if needed. Our team reviews a summary of each of those calls the next business day and reaches out to families who need follow-up.

PCMH means constant improvement.
